Apply Job Type Full-time Description Assists the Case Management staff by performing clerical duties related to case management and discharge planning.  Has full access to patient health information. Assists with coordination and completion of forms including but not limited to: FL-2 forms - CM staff review and doctor’s signature for extended care facility placements Important Message from Medicare - obtaining patient/family signatures prior to discharge As needed, takes documents to physician office for signature and/or delivers document to post-acute providers Assists with coordination of information such as: Discharge date, disposition, discharge instructions/orders and/or discharge summary to post-acute providers Communicates pertinent patient information to post-acute providers and 3rd party payers.
